Day Habilitation Support Services
CWC's Children’s In-home Day Habilitation Services are “personal coaching” services provided to a child/adolescent (consumer) in his/her home or community setting. The focus is primarily on behavior management, skill acquisition and physical development activities. The goal of these services is to achieve an increase in a child's level of functioning, an increase in skills development and a decrease in maladaptive behaviors. CWC encourages the active involvement of the consumer’s family, parent/guardian and the family's natural supports in this process.
These services are provided under the direction of a professional staff person with the skills, training, education and experience sufficient to meet the expectations of DHHS’ high standards. Support staff will meet the qualifications required for the Habilitation Specialist position.
Services Provided
The team works with the child and family and other providers to:
- Collaborate with the child’s family, school, therapists, caseworkers and any other pertinent individuals to identify strengths and needs.
- Develop a comprehensive psychosocial assessment
- Facilitate the individual program planning process.
- Develop an Individual Service Plan for each child with a minimum of quarterly updates.
- Match and assign a Habilitation Specialist to work with each child in the least restrictive setting available.
- Design programs to match the health, safety, and human rights requirements of the state of Maine and Federal statutes.
Service Mix
Habilitation Services designed by the child’s treatment plan are provided to the child by his/her Habilitation Specialist in the child’s home or a community setting, and include but are not limited to:
- daily living skills
- behavior management
- coping skills,
- time management
- appropriate social interaction skills
- community integration skills
- increasing self-esteem/self-confidence
- following directions and staying on task
- developing and maintaining the child's sensory, motor and psychological skills development
- physical development activities to promote self-maintenance, self-awareness, and self-motivation
Staff Qualifications
A Habilitation Specialist provides direct care day habilitation services and must have a minimum of a high school diploma or equivalent and at least one year of documented relevant experience.
All CWC Habilitation Specialists receive certification through the Habilitation Specialist Certification Course. All CWC's Habilitation Specialists undergo a motor vehicle, child protective and State Bureau of Identification background check.
CWC complies with DHHS and MaineCare requirements in regard to training and hiring for provisional and certified workers.
Eligibility
Children and adolescents qualify as follows:
- Child is birth to age twenty.
- The child/adolescent must have mental retardation or
autism as his/her primary life challenge.
- The child has a functional impairment in significant areas of development.
- Children will meet the above criteria and are in need of services due to:
the child's behaviors,
physical, sensory, motor or psychological impairments,
mental retardation, or autism and their needs cannot be managed
solely by the parent/guardian,
and
- The child is within the area of CWC coverage.
- CWC has the appropriate trained staff available for the primary diagnosis.
- The referral is within CWC's program limitations.
