Shared Living Options

The Charlotte White Center has been providing social services for nearly three decades. Throughout our history of service we have developed programs of support in partnership with individuals living with the life challenges of intellectual or developmental disabilities. We now offer customized living arrangements and supports to adults and children with these challenges who are seeking greater control over their living arrangements. This residential service is based on meeting individual needs and preferences.

Shared Living Options
Two Models:

Host Families

The Charlotte White Center contracts with a “host” to provide a home for an individual within a family environment. The host can be a single person or family. Hosts may already be involved with the individual as a friend or even as a family member. The Charlotte White Center will qualify the home as a provider. In order for the placement to be successful it is crucial for an appropriate match to be made between the host and the individual. Individual supports offered under this model can include respite, direct care supports, training, and collaboration with the Charlotte White Center.

Eligibility For Services

An individual seeking residential support is eligible for this service if they meet certain criteria:

  • A person 18 years of age or older and
  • Has a diagnosis of a developmental or intellectual retardation
  • Eligible for DHHS funding (MaineCare waiver)

Once eligibility is established, an interview to determine the individual’s needs and desires will be scheduled. The team will seek to create a match with providers who best meet those requirements.

Referral Process

We encourage referrals from individuals, family members, the medical community, case managers, or service providers. Once the referral is made, a time for an interview will be scheduled. The intake involves gathering data necessary to determine the best living arrangement.

How to Become a Provider

Interested individuals or families can become a Shared Living Host by contacting the program director. An application packet will be sent which will gather personal information to help us get to know each other better. Once the necessary paperwork is completed and returned to the Charlotte White Center, an interview and a site visit at your home will be scheduled.

The interview and matching process determines the best person to provide support in your home. Additional information is then gathered regarding program expectations; motor vehicle, adult and child protective background checks and references.
